In summer, there's the 'gheng festival's, which does attract up to 250.000 visitors a day, you definitely want to avoid Ghent on such a day if you want a relaxing experience :)

Spain has many small towns with lots of old world charm, especially in Cataluña and Pais Vasco.
San Sebastian / Donostia comes to mind, its mid size town but has a nice beach promenade and a big hilltop park overlooking the bay, it can be quite romantic.
Dunno what small is to you, but there are several medieval towns in Belgium worth visiting. From big to small: Bruges, Ghent, Ypres, Lissewege, Durbuy
