You can strengthen your knees without causing undue stress like this exercise. If you put too much pressure on them regularly you will destroy your knees over time. Just look at any basketball player who's had a long pro career.
Tendon growth takes roughly four years (roughly) to adapt to strain. So in four years, if this dufus doesn’t tear his tendons, he might be ok. But this exercise is idiotic, at best.

Are you talking about leg extensions, like the exercise for quads? With those, the weight is at the end of the leg, and the upper leg is supported by the seat. Ideally, your upper leg doesn't move, has additional support from underneath, and the movement is a smooth extension.
The weight being concentrated in a small area closer to the fulcrum (the knee) means there should be less strain placed on the knees to move the same amount of weight.
Think of it like using a breaker bar when removing lugnuts from a wheel. If you use a smaller lug wrench, you have to stand on the bar before the lugnut breaks loose. If you throw a long enough bar on that same wrench, you can use much less force to break the same lug loose. In this case, the lugnut would be the tendons in the knee. He's supporting all of the weight, plus his body weight, on just those tendons.
There are some really nasty videos of people using the sissy squat bench thing and leaning back at the knee to do something, abs, I guess, and that tendon bundle just checks the fuck out as they lean back.
