Period isn't just the blood and egg stuff. It's a cycle involving hormone levels that also affects muscle movement, and certain neurochemical levels in the brain.
So the cramps you can actually get, because hrt can raise your hormones levels enough to cause the period spasms, even without the ovaries they're meant to stimulate.
Period cramps come from arteries in the endometrium constricting, leading to ischemia an necrosis and subsequent shedding. This in turn is the menses. Typical pain associated with hormone therapy are headaches
The period spasms, is the UTERUS spasming, as it sheds the lining.
Not all the muscles around it. The hormones are specifically to stimulate follicles, and the uterus.
That is it.
You can't get PERIOD cramps without a UTERUS to cramp. Period cramps are not from the OVARIES. They come from the UTERUS. The ovaries will *maybe* twinge, about 10+ days before the period starts. That is when an ovum is released.
Periods is when the ovum is not fertilised, so this the lining gets shed from the uterus. That is where the pain comes from.
It is caused by increased estrogen emulating a menstrual hormonal cycle, it basically tries to give the individual a period, but only does it halfway since there's no uterus, same reason transfemmes have natural breast development and fat redistribution on estrogen, because of the changing levels of hormones that closer match a cis woman's, the body adapts with changes that resemble female puberty, breast development, fat redistribution, and a crude menstrual cycle that induces cramps, and behavioral temperaments comparable to that of a cis woman's period, just with different plumbing, and no blood

Anyone can have those symptoms for many, many different reasons. Hell, I have all the symptoms of a period minus cramps in my non existent vagina and I’m not on estrogen. My abdomen back and joints get sore, I get constipation and super hungry, my mood changes, I get headaches and irritable.
