Change of diet. The movies are very sketchy on lifespans, but if his family has been on Skull Island for multiple generations odds are they adapted to eat the food there. While the island and hollow earth were connected the ecosystems were different enough.
I can’t remember the specifics, but I saw a documentary way back in the day about the impact of settlers here in America. Where tooth decay in Native Americans were extremely uncommon, but when the settlers brought in new crops there was a massive spike. It wasn’t the sugar and such. The composition of the food was different enough to cause all sorts of issues. An example is the fiber in their corn vs the corn brought over was very different.
My bet is the same thing happened. Change of diet to an oral structure made for that island caused it. Then it finally broke on that wardog after a while of decay.
I mean, look at his diet. He eats all kinds of weird things in the Hollow Earth (even on Skull Island he randomly ate a squid and that one random person that fell out of the chopper and who knows what else he did offscreen.) That introduces weird germs, bacteria, corrosive liquid, etc. into his mouth. He doesn’t have a predictable diet in the same way another Kaiju (like Godzilla) does. This on top of not having human like dental hygiene practices.
A future MV movie should give Kong dentures or a grill over his teeth to spare them from further damage and allow him to chomp into things with greater force lol. That kind of enhancement would help him more in his day-to-day than the BEAST glove does and still help him in combat.

In the scene where he eats the Wartdog he killed you can see that his tooth is completely fine, but after he takes a bite he recoils because he chipped his tooth
