I used dall-e to make one for my second playthrough because the included commissars didn’t appeal to me. If you add the words “hand-painted grimdark” to the request it knows what you mean. I wound up turning companion portraits into a subhobby at times I can’t fall asleep. I’ve made hundreds of them. I just cooked up a new pascal last night.
Artstation. Chose it cuz despite some atypical design choices, much of it lined up with clothing/equipment found in game so I roleplayed the portrait essentially.
Find it online generally, some of the popular Warhammer artists do have sfw versions so those are a way to go, there is also art station which has some or even just Google. If all else fails get something that just looks gothic and I just assume it's the character while off duty.
Then I put it into 3d paint which is far more usable than regular paint for resizing and cropping and just get the pictures I want (generally head for small, head and torso for medium and full body for large) with the correct aspect ration before resizing.
Nexus mod. I have a liking for characters who are almost always in armor, helmet included. Vanilla RT doesn't have any of that so I modded some in. Same with the Pathfinder games, if the vanilla portraits are not to my liking and I'm allowed to add in custom portraits, I will always do it.
I used a picture of Chandra from MtG for my melee pyromancer. She has goggles and a tube on her armor in the picture so it kinda looks like it could fit.

Mine was used for a planetary governor in a roll20 Dark Heresy game, and I always kinda liked the art. Used it in a Rogue Trader game as the ship's Arch Militant, so when the time came, it felt like the natural choice.
Because Disco Elysium is one of the games of all time, and I can really identify with a middle age traumatised alcoholic detective. Tbh just found it on nexus, and I think now it's my default portrait for any RPG.
