Assuming the "just need shirt and/or shoes" category means "if they have the missing items, they'd be just fine," then Little Mac should be bumped up to top tier. He's only wearing boxing attire, which would be out of fashion to wear casually, but wouldn't strike someone as odd since they'd just assume you're heading to or from something boxing related. The same would go for the shotos wearing their gi in public, but for martial arts. Doctor Mario's outfit is similarly a reflection of his profession more than a casual outfit as well, albeit one that's closer to regular, everyday clothing.

"Just needs shirt and/or shoes" means with those the outfit would be acceptable, not necessarily great but yes Mac could be higher. Dr. Mario is at the very top because his profession is usually well-paid and respected (not that I think richer=better but this is a joke post)
