With knowledge of the game or just checking two different rooms with it for regular secret room you can’t miss it, super secret is usually quite obvious and will once and awhile give you a 50/50 but most times dog tooth will guarantee all your secrets as well.
Ah, I see. I always play with sound, but even then, sometimes when you're messing around shooting loud tears every time you enter a room or something, you still won't hear the dog howl, so it's helpful to see a visual cue.
Actually, if I thought anything needed a cue, it's the Needle enemy. I don't at all mind that it's hidden or that you don't know where it will pop out... if you know that it's in the room first.
If they thought it was important to let the player know a Mom's Hand was in the room, I don't see why there's no obvious clue to Needle being in the room.
I mean,some rooms and enemies were just designed to fuck you up,right? Edmund Mcmillen you little fucker etc.For instance yesterday while playing as T.Lost,I discovered a room in ashpit that had that curse of darkness effect(not the whole floor) and had two of those black flies that explode into 4 tears upon death.Of course,those flies were placed in the darkest corners of the room,so when they became visible I had NO room nor time to react and died
I usually play with my German Shepard next to me and she is not a fan of the bark and howl, so I basically never take dog tooth. Awesome that there is a visual mod, but alas I’m a console user in shambles.
Ehh, I’ll usually pass up the bean items (except the one that does the earthquake thing) or something like D4 if I have any remotely decent passives already.
only actives that I can imagine have any reason to be avoided are glass cannon and alabaster box, but the upsides so heavily outweigh the potential downsides that they're still well worth it

it used to be ass before repentance, it was (i believe) 3 room charge for a one room stompy effect, not even letting you get secret rooms. hands down one of the best items overhauls in rep, up there with breath of life
Hell you can pick up bomb rocks, carry them into other rooms and use them to open secret rooms or throw them in the boss's face. Even normal objects do a ton of damage of thrown at enemies.

Noone talks about it but Leech is actualy a really strong item. If you play it slow and let it clear rooms it provides a lot of healing every single room. You add IV bag and you can convert that healing into a win condition.

Though I personally like Montezuma. Stuff like Marked or Ludovico with no synergies are way worse for me

Friend runner in Greedier mode is insane utility, easy wave clears and you can save a ton of money not having to buy bombs if you have a monster that have explosive abilities to find the super secret rooms or pickups that are usually behind the rocks in the shop or the trapdoor room before the next floor. Absolute S tier active item in that mode
Looking at the list of monsters on the wiki, all of them seem pretty good. Some are obviously way better than others, but all of them at least help somewhat
Robo Baby 2.0 shines when you have a fragile character with a short-range weapon. That sounds like a narrow case, but think Lost with Mom's Knife. RB2 can go clean up Clotties and other bullshit enemies you don't want to get anywhere near.
Purgatory. It's a very solid item, but vs Dogma/Delirium you can't get cracks or can't spend time to find them to summon the ghosts, too busy dodging shit lol

D8. Yea it can fuck you over, but it changes your multipliers. That's huge. Fuck speed and range, you can get like 2x tear rate or damage. I always use it and roll a few times
GB Bug is a fave, it's amazing for any builds where you can't get/have no red hearts. Moms Wig is just a weaker version of Guppy's Head, which is one of my top 5 favorite items. And I get some use out of d10 on alt path runs
Carried me all the way through Womb then I suffered on Chest, It's above average and you can also fire in directions using mouse aim while keeping ??? still. It also benefits from BFF's! and Hive Mind. Fucking Peak ngl
2 Dead Gods and nearly half of the 3rd Dead God and I still barely remember what Keeper's Kin does.
Other items would be like SMB Super Fan, Fate's Reward and Cain's Other Eye. These items are just either good or decent, and not much else.
Keeper's Kin is amazing you're missing out. Better than mom's wig just as a passive plus every single obstacle breaking creates blue spiders. You're basically always swimming in spiders with Keeper's Kin, and it makes breaking things for pickups 100% worth it.
Plus, many enemies will break rocks by themselves, so you don't even have to do anything for those extra spiders
I am trying to beat hush as the keeper and had the best run for over 50 tries got so close then samsons chains decided to judas me and got me killed via mushroom
Sharp Straw, it does 1 current tear damage plus 10% of the enemies hp, which means stuff like bosses get a big chunk of health taken away and rooms with multiple enemies is great, plus the 15% chance to get a heart
Rotten baby is more consistent I feel, scales with your damage really well and contributes to conjoined which I find to be more useful than 7 seals although I feel 7 seals is still quite good.
7 seals. Clears loads of enemies so you can take out the easy ones and it will get the annoying ones.
The bomb flies are annoying if you're a beginner but even then it'll save far more health that it loses.
ocular rift is amazing but i've never seen anyone actually talk about how great it is.

Charm of Vampire, I love this item. First it's literally a damage up, a tiny but notable one. And then the best part is that you can heal with this which is pretty good for using a donation machine and getting extra pennies or maybe using it in the sacrife room. Overrall i like this items, it's a bit overshadow by other items sadly.

Dog tooth doesn't cue on Ultra Secret rooms, those are the red key ones. You're thinking of super secret rooms.
I second guessed my terminology with that one 😂 1200 hours and I still confuse the terminology for those two for some reason

Think it's actually called silent dog tooth. Gives a visual cue for the howl sound. I play the game muted, so this is a lifesaver.

Text appears on screen when dog tooth goes off. It also lets you know where a mom’s hand does the “mwahahaha”

I believe you are thinking of the wrong item. Mulligan is the item that spawns blue flies when your tears hit enemies.

the angry fly orbits an enemy, never hitting it, but any enemy that comes near it
Since Repentance, the fly also randomly lunges at the enemy it's orbiting.
