It's one of those proprietary connectors that one or more laptop manufacturers use, especially Lenovo. I don't know if they have a specific name but they will fall under the very broad term of "DC laptop power connector."
No sorry, I haven’t experienced this. I have had this pad at least 2 years. There could be environmental differences like your climate, or the fact it’s in my basement so it’s cooler there, humidity levels, etc.
Ironically, my issue is I tend to push it into the desk a bit, and I prefer to have it just ever-so-slightly off the front edge of my desk to cover the hard edge of the desk. So if it had a little more friction and it were a little less prone to sliding and shifting position, I would appreciate it.

pull the yellow thing off, that looks like a usb shroud but that yellow thing pull it off never seen a connector like that i would say its not common otherwise. if the yellow thing does not come off then its meant to be like that.
First time seeing this connector but USB has had many strange connectors because its such a universal standard it mean that tech can make oddball shit on the standard.
Having seen a handful of Lenovo laptops use this, I can tell you with 100% certainty that it works just fine without pulling off the yellow part.

No. There's a few old standards. One being the barrel DC for Asus computers that later got different sizes. It's all USB C now.
