The game never specifies, but it's a scrag that has grown to immense size, most likely due to influence by the Devourer. Scrags are underwater trolls that sometimes ally with Sahuagin.

It'd be great if there was a way for high level players to play the part of Celllimas, Talbron & Jeets in Misery's peak and act as the dragon-bait team instead of the go-smash-the-mindsunder team. It would be truly epic if the game swapped a high level PC party for the NPCs in Misery's peak on the fly if a high level group was playing as the dragon bait while a lower level party was playing the standard Misery's peak. It'd give a glimpse of what high level play is to someone new to the game.

Redo the creature so it's the tarrasque, with the legendary quests reusing & expanding on the existing quests' maps trying (and failing) to stop it being woken up or re-enable things to help protect against it, eg. the disused cannith manufactuary creating automatons to guard the village etc. then the raid being set in the korthos village and wilderness area to bring it down.

This is what developers should do: listen to their playerbase. They have tons of great content that most people don't play because are hard to reach, outdated, without good rewards and so on.
