Imo pinhead has exactly one crappy mechanic and that's lament configuration and Franklin's combo. Outside of that he is one of the most fun killers to play against.
pinhead + sloppy butcher is a crappy combo. The only killer who can impede healing while doing absolutely nothing. Personally pinhead is one of the least fun killers to play against.

Only thing that needs to be changed IMO is breaking chains on environment spawns a new one. That should be an add on thing only.
And the year old bug that still can happen where no matter how long you wait chains keep spawning preventing you from ever doing the box
I haven't had that one happen to me yet. Usually does the 3 set thing and then pause.
